Question : Evangelism follow up form

hi there. I am in charge of a church youth camp follow up ministry in that if someone decides to accept the Lord or make a recommitment or some other action to have a form available so that we can do follow up with the if you know what I mean...

There will be different churches there and I am thinking it will be best that each church will be responsible for following up with their people..

I need to design a form....any ideas for this and how to go about it?

if you have sample forms that will be good...Im thinking about using word but what do you think...
