Question : How do I stop blank orders from being submitted

I created a database to hold the orders we recieve off our website but for some reason we keep recieving balnk orders. I am using cPanel X and created a MYSQL database to hold data off the form from the site and also wrote an .inc page to send the orders to us via email. I created the form to require that all info be filled out to submit and have tested that and have no idea where the blank forms are being send from or why they see to be random. If anyone could shed some light as to where I should look I would appreciate it, thanks.
