Question : Particular calendar items will not sync from iPhone to Outlook via Exchange

Exchange 2007, Outlook 2003, and iPhone 3G. Mail, calendar, and contacts sync perfectly well except for one calendar item he added to his iPhone days ago and it still has not shown up in Outlook. This happens to perhaps one calendar event per month. What can I check or try when this happens?

Is the person using the exchange calendar as the only calendar on the phone?  I have seen many times where the calendar event is being created on a personal calendar on the iPhone rather than on the exchange calendar, hence the sync issue.
