Question : Display data from fields in multiple tables on a single report

Hello Experts,

    I have been struggling with this for some reason.  I thought I had done this in the past and don't recall it being this difficult - so a little help would be much appreciated!

    I have an Access 2007 DB. I have a series of tables that are relationshipped as shown in the attachment.   I am attempting to write a report based on a query built off the assessments table.  In that report I would like to display data from all four of the displayed tables.   However, when I set the report up I am only able to get ID #'s or nothing at all to display. Every attempt to change the ID being displayed to show related content results in blank display.

    I am sure I am missing something simple but at this point need someone else to point it out to me...

Which ID drives the report as there are 4 tables? Also I note that in your ER diagram that you have a Practice_ID field in the practice table but the primary key is the Practice_Name field.
Is this intentional?

Did you enforce relationships between each table?
