Question : Acrobat PDF forms

Is it possible that when you go to a PDF and read the first page, to then place a pop up form (or embedded form) of some type if the user wants to read page 2 and so on?  This way, if they like what they see and want more information on the subject, we can gate the rest of the PDF pages with a form.  Does anyone know how to do that?  Is there something in Adobe Acrobat that can do that, or another available method?  And if it is possible through Adobe Acrobat, where would the user's information from the form go?  Can I designate an email address for the information to be sent to?

I'm not sure about the first part but Acrobat can create forms quite easily and submit to email or database.
I use this method for registration of the software I sell. I actually found that people have an easier time than with an online form. I have no idea why but have got less questions / calls since I switched to PDF form.

One way of doing this would be to create an email address with an auto-responder that sends them another PDF. This way, the user would submit the form to that email and instantly get another PDF file from auto-responder.
