Not running 11.5 but had the same problem with 12, try this:
1. Start the Report Writer and enter the name you want to use for the template in the Report Title field.
2. Select the objects for the queries and columns to be included.
3. Click Next to go to the Report Criteria page and if desired add any Criteria.
4. Click to save the report.
Note: If you added criteria it is a good idea to first generate the report to verify the output is as desired. Once you are satisfied with the output then click Back and save the report on the Report Criteria screen. When creating another report be sure to change the name in the Report Title field at the Report Query page. This is needed because it is the name used in the Report Title field for the template name shown under Reports > My Reports and not the name given when doing the File Save As.
5. Go to the Reports window to view the template under My Reports. If the Reports Windows was already open then right click on My Reports and select Refresh to enable the template to show up.
6. Right click on the template and select Create Now or Schedule to generate a report and have it show up under My Reports.
