Question : Best practices for connecting to mysql?

we have survey but a lot of users complain that at the end of the survey the data is not getting stored. I am wondering if the developer is handling the connections between php & mysql properly

Is there a link to best practices taht need to be followed for php code to connect properly (connection pooling, detecting connection errors etc)? Like a check list we can run through to ensure we are doing it right ?
