Question : Lookup

I want to produce a Result dataset that contains one line for each User in the Source dataset and returns the Value associated with each column in the Result dataset.  If the Source dataset does not contain a Value for any given column in the Result dataset, then the given column is empty in the Result dataset. (SEE EXAMPLE BELOW)
