Question : creating report from combo boxes

I have attached a database.
open database
a form opens
I have 3 combo boxes - if you open the report on form -
you will see my problem
I want to be able to create report with name, part# and part description.
Are my data types wrong? or something in properties of combo boxes?
I  tried joining tables and cannot make it work.
I just get numbers displaying in name and part # fields.
