Question : doc for code review

I need to put a reference for a small app that is sitting locally, but will be deployed to public soon

- basically for every screen, need to put down
*input & path (what methods, web services etc) to get the output
*path - from here to there back to result (as an example)
*output (what result set)

is there an official name for this kind of documentation? would there be any sample document of this  nature to refer to, to make it professional.

Could you not insert that info into standard Javadoc?
