Question : dropdown list that passes to php var

I want to have a drop down list on a page.  When a user selects something from the drop down that value is saved in a php variable ($card).  So when the form on the page posts it sends the variable with it (in a hidden input).

I only have access to this one page.
The page the form is posting to is paypal.
