Question : database for countries, states, cities and if the city is from usa then zip code

Hi,

I want to make registration form with the details like country, then select state then select city. All the things should go in order.. And if the city id from USA then user can fill the zip code.

I want this in ajax and also where can i get the database of all the countries, states, cities and zip code of usa?

In which case, I wuold delete and re-create the profile in case it has become corrupt.
